Appointments can be made by telephone or at reception during surgery opening times. Please tell the receptionist if you will need an appointment longer than 10 minutes. Please let us know as soon as possible if for any reason you cannot keep your appointment.
Please phone before 10am and tell the receptionist if your problem is urgent and you think you need to see a doctor on the same day.
We are taking patients from a catchment area. All new patients are offered an appointment with the practice nurse before joining and will be required to fill in the relevant forms. Bring your medical card, proof of identity (e.g. passport) and proof of address (e.g. council tax bill) with you. All newly registered patients need to have a full health check with the nurse before registration is complete.
Please fill in this form and bring it to the reception to register with the surgery.
